GOC registeration for adaption course

I am a Bachelor of science in Optometry graduate from India. I am currently working as a full time optometrist in India itself. I was looking forward to join the optometry adaption course provided by A.R.U. So it is shown that I should have a GOC registeratio for the course and moreover I should get a letter reccomending me for the course. I would like to know more about this. Also when I attempted the qualification check I was told to submit a consent form. What is this consent form?
